The compliance trap
Traditional HSE models focus on compliance: are the documents in place? Are the inspections done? Are the KPIs green? This creates a dangerous illusion of control.
Compliance tells you what should be happening. It doesn't tell you what is happening. In complex projects, the gap between work-as-imagined and work-as-done can be enormous — and that gap is where incidents happen.
KPIs that mislead
Common safety KPIs — lost time injury frequency rate (LTIFR), safety observation counts, training completion rates — measure activity, not effectiveness. A site can have excellent KPIs and still be drifting toward a serious incident.
- High observation counts may mean people are filling forms, not learning
- Low incident rates may mean people aren't reporting, not that the site is safe
- Training completion doesn't mean people understood or changed behaviour
What safety needs instead
In complex projects, effective safety requires:
- Insight into work-as-done — understanding how work actually happens, not just how it's planned
- Weak signal detection — noticing the small things that precede big events
- Adaptive capacity — the ability to adjust when conditions change faster than plans
- Leadership connection — leaders who understand site reality, not just dashboard summaries
- Learning from normal work — not just from incidents, but from everyday successes and near-misses
